For all SUNY institutions, you will need to complete a Participating Institution Agreement (PIA). Please contact the Director for a 2020 PIA. Once signed by your campus’ authorized signatory, you are able to submit the completed PIA directly to studentconductinstitute@suny.edu.
For all other institutions, please complete the form below:
To maintain uniformity and consistency of the legal obligations required for membership with SUNY SCI, modifications to the posted terms and conditions are not permitted. That said, an exception may apply to any organization(s) that is public institutions and/or state agency. Any such entity must provide written documentation to support this status for consideration, which may include, statutory citations, Articles of Incorporation, charter, funding source/budget documents, etc. When a potential member requests such an exception, the applicable legal/statutory citation(s) demonstrating the necessity for any requested change must also be submitted. Lastly, please provide your attorney/Legal Counsel’s contact information (Name/Phone/Email) to discuss any such requested modifications.
NOTE: SUNY SCI reserves the right to decline any requested modifications to its terms and conditions that are not required by applicable law.
